Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Often, treatment can be done the same day as the consultation. A complex medical history or treatment plan, however, will require an evaluation prior to your treatment appointment.
Please assist us by providing the following information at the time of your appt:
- Your endodontic referral slip from your dentist. You may bring copies of X-rays, however, we may take additional X-rays at various angles to aid us in our diagnostic evaluation.
- A list of medications you are presently taking.
- If you have dental insurance, please bring a card if provided or necessary account information. This will save time and allow us to help you process any claims.
All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation and treatment visits. Furthermore, they must remain on the premises throughout the entire procedure.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.
